Original Medicare vs. Medicare Advantage

Original Medicare (Parts A and B) is the federal health insurance program for people 65 and older or those with qualifying disabilities. Part A covers hospital care and Part B covers outpatient services, but Original Medicare has no out-of-pocket maximum and leaves you responsible for 20% of most costs indefinitely. Medicare Advantage (Part C) plans are offered by private insurance companies approved by Medicare. They bundle Parts A and B and often include additional benefits like vision, dental, hearing, and prescription drug coverage. Most Medicare Advantage plans have $0 premiums and include an annual out-of-pocket maximum, providing greater cost predictability.

What is Medicare Supplement (Medigap) Insurance?

Medicare Supplement plans, commonly called Medigap, are sold by private insurance companies to fill the gaps in Original Medicare coverage. Depending on the plan letter you choose (Plans A through N), Medigap can cover your Part A and Part B deductibles, copayments, coinsurance, and even emergency care during foreign travel. Unlike Medicare Advantage, Medigap works alongside Original Medicare — you can see any doctor who accepts Medicare nationwide, with no network restrictions. Plan G is currently the most popular Medigap plan for new enrollees.

Medicare Part D — Prescription Drug Coverage

Medicare Part D is optional prescription drug coverage sold by private carriers approved by Medicare. If you have Original Medicare or certain Medigap plans, you'll want to add a standalone Part D plan to cover your prescriptions. Medicare Advantage plans often include drug coverage built in. The plans vary significantly in which drugs they cover (the formulary), what tier your drugs fall on, and what your monthly premium and copays will be. When Can I Enroll in Medicare?

Your Initial Enrollment Period (IEP) begins 3 months before your 65th birthday month, includes your birthday month, and extends 3 months after — a 7-month window. If you miss your IEP, you can enroll during the General Enrollment Period (January 1–March 31 each year), but late enrollment penalties may apply. The Annual Election Period (AEP) runs October 15 – December 7 each year and allows anyone on Medicare to switch, drop, or add plans for the following year.
